Are you a clinically savvy relationship-builder who thrives at the intersection of care coordination and innovation? Join myLaurel as a Clinical Liaison and help bring hospital-level care directly into the homes of medically complex patients across the Greater New Orleans area.
We’re launching with a major health system partner—and you’ll be on the ground floor of a care model that’s redefining how, when, and where patients heal.
Build Relationships : Partner with hospital teams, physicians, and case managers to identify eligible patients
Drive Referrals : Develop and optimize workflows that support seamless hospital-to-home transitions
Engage Patients : Educate patients and caregivers on our model, secure consent, and support discharges
Leverage Clinical Expertise : Apply sound judgment to help determine patient eligibility and needs
Collaborate Cross-Functionally : Work closely with internal teams to ensure exceptional care continuity
Measure Impact : Track referral metrics and patient outcomes to drive continuous improvement
Clinically licensed: RN, LPN/LVN, LCSW, or EMT-P (required)
2+ years of experience in inpatient, acute care, or ED settings
Proven skills in discharge planning , care coordination, and navigating complex systems
Strong communicator and relationship-builder
Organized, proactive, and metrics-minded
Tech-savvy with EHR experience
Must have reliable transportation
Valid driver’s license and auto insurance
$90,000–$105,000 base salary + productivity bonus
