Job Description:
The RN Case Manager position will be responsible for the following duties at Adena Pike:
Navigating the swingbed needs/discharge planning
Facilitates transition planning across the care continuum by involing the patient, family, providor, and other healthcae team members.
Works with patients/families to promote self- management support of chronic diseases through individualized plans with short/long term goals.
Ensures suspected cases of abuse, neglect or exploitation have been referred/reported to the appropriate parties or agencies.
Required Certification: BLS
Required Experience: RN Case Manager
