Position Overview:

We are seeking a dedicated and reliable Licensed Vocational Nurse (LVN) to join our growing team. The ideal candidate is patient-centered, detail-oriented, and passionate about making a difference in people’s lives.

Responsibilities:

• Provide skilled nursing care in accordance with the physician’s plan of care
• Monitor patient health, record observations, and report changes to the RN Case Manager or physician
• Administer medications and perform treatments as prescribed and as needed 
• Collaborate with multidisciplinary team members to ensure the best patient outcomes
• Educate patients and families on proper care techniques and medication management
• Maintain accurate and timely clinical documentation

Qualifications:

• Current and valid California LVN License
• Minimum of 1 year of clinical experience (home health experience preferred)
• Strong communication and interpersonal skills
• Compassionate, professional, and dependable
