LVN - Field Nurse Home Health
Around the Clock Home Care - Bakersfield, CA
Job Description:
Our LVN's provide and organize nursing services for Home Health Agency patients. This includes both home evaluations, assessments to determine the plan of care, treatment, and teaching under the plan of treatment.

Key Responsibilities:
Regularly assess and re-evaluate patient's nursing needs and develop Plan of Care. Develop care plan which establishes goals and incorporate therapeutic, preventative, and rehabilitative nursing action. Prepare clinical and nursing progress notes and informs physician and other personnel of patient's needs, change of condition. Obtain and receive physician's orders as required. Works with patient and caregivers in nursing procedures, education, and related needs appropriate to diagnosis and plan of care. Actively participates in coordination of health care services and case conferences. Attends and participates in in-service training and compliance programs.
Minimum Qualifications :
Maintain confidentiality of all confidential information. Knowledge of Care Planning and treatment techniques. Ability to deal with diverse personalities and cultures on a routine basis and in a professional manner. Meet deadlines and maintain ongoing timelines to accommodate Home Health agency goals. Communicate effectively in both written and spoken formats at a professional level.
Experience and Education :
Any combination equivalent to education and experience that could likely provide the required knowledge, skill, and ability will qualify. These would typically be: A Licensed Vocational Nurse with one to two years of nursing experience. Able to provide two professional references of LVN work experience.
Requirements:
Must have at least 1 year experience
Possession of valid California Drivers License: Class C
Valid automobile insurance
Ability to pass required physical, drug screen, and background check
Provide proof of current licensing.
Provide current first aid and CPR cards.
Must possess IV certification
