Location: Austintown, OH | Employment Type: Full-Time | Compensation: $45/hour + Bonuses

What You’ll Do

As a key member of the care team, you will:

• Conduct comprehensive psychosocial assessments for residents with cognitive challenges, dementia, or other neurological conditions.
• Provide individual and group therapy to enhance emotional well-being and cognitive resilience.
• Create and implement personalized care plans, collaborating with medical professionals and caregivers.
• Offer family counseling and support, helping loved ones navigate cognitive health needs.
• Partner with neurofeedback specialists, physicians, and nurses to deliver coordinated, whole-person care.
• Maintain compliant, timely documentation in accordance with Medicare and insurance guidelines.
• Educate staff and contribute to the development of protocols and best practices for cognitive support in assisted living.
• Lead support groups and promote resident engagement through cognitive stimulation and therapeutic activities.

This is not traditional social work in a facility. You’ll be joining a forward-thinking program redefining how cognitive care is delivered to aging adults. Our interdisciplinary team combines clinical expertise with compassion, using both high-tech and high-touch approaches to support residents at every stage of their cognitive journey.

What We’re Looking For

• Master’s Degree in Social Work (MSW) from an accredited program
• Active LISW license in Ohio (or ability to obtain)
• 2+ years of clinical experience—preferably in geriatric, neuropsychology, or cognitive health settings
• Familiarity with neurofeedback, CBT, or other cognitive/behavioral therapeutic approaches
• Working knowledge of Medicare/Medicaid billing for behavioral health
• A collaborative, ethical, and compassionate clinician ready to make a difference
