Ideal Care Home Health is an established home health agency, serving the Southern California communities for nearly 18 years. Currently, we are seeking reliable per diem home health LVNs to provide home visits for home bound individuals. The ideal candidate must have minimum of 4 full days of to dedicate to our company. Sign on Bonus!

Also looking for weekend only nurses

Qualifications:

• Graduate of an approved program of vocational nursing
• Licensed as a licensed graduate practical nurse in the state of California
• Minimum One (1) year of experience as a nurse
• Home health experience highly preferred

• West L.A. and surrounding areas
• East L.A. and surrounding areas
• South L.A. and surrounding areas

Responsibilities:

• Participates in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of treatment
• Follows the nursing care plan for each assigned patient
• Observes the patient; evaluates the care of the patient; applies her/his knowledge of nursing skills to the patient assigned
• Accurately reports and records the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepares clinical/progress notes on the day of the visit and incorporates same in the clinical record weekly
• Prepares summaries in cooperation with the RN supervisor.
• Assists the patient with activities of daily living while teaching appropriate self-care techniques
• Provides and maintains a safe environment for the patient
• Provides optimum physical and emotional environment
• Must submit clinical notes with in 5 days of visit
