Every day patients come into our clinic overwhelmed with navigating complicated healthcare systems, and chronic diseases. The Health Coach comes in to improve a patient's experience at Saint Thomas, and in the best scenarios, arm them with tools to make positive life changes. Health Coaches are afforded the opportunity to work closely with providers, nurses, medical assistants and case management. The role includes developing relationships with patients, reviewing care plans, and setting goals. The health coach offers supportive guidance to help patients foster healthier lives. We are looking for candidates that are compassionate, good listeners, kind, and skilled at problem solving.
