Compensation: Salary plus a bonus structure, processed bi-weekly.

Shift Type: Full-time with participation in backup supervisor rotations.

Summary

The Clinical Manager directs, coordinates, and evaluates the delivery of home health services to a team of clinicians ensuring compliance and consistency with agency policy and State and Federal home health regulations. This position takes the lead in attaining the team’s clinical and financial objectives.

Responsibilities

Oversee all patient care services and personnel for a clinical team including coordinating referrals and patient care, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the patient’s individualized plan of care.

Assists in policy review and revisions as requested to establish and support the highest possible quality of patient care, cost controls, quality assurance, and staff performance.

Provides and oversees orientation, evaluations, and continuing education for personnel, as well as other contributions in support of patients, personnel, and the agency.

Participates in the QAPI plans and processes, including the review of Plans of Care. Ensures follow-up on any required action plans.

Reviews weekly dashboard report to ensure the organization makes regular progress against its strategic goals and addresses issues raised through metrics.

Leads weekly case conference and addresses recert / discharge decisions based on case conference findings.

Supervises the scheduling process from referral through discharge to ensure that clients receive assessment visits and that these visits are scheduled and performed timely per agency policy. Ensure that referrals are appropriate for home health.

Serve as a liaison for in-field nurses and clinicians. Help troubleshoot and provide support for clinicians. Perform patient visits if necessary.

Qualifications

Registered clinician with two (2) years in community health/home health program.

Previous supervisory experience is a plus.

Bachelor’s degree in a related field preferred.

License and current registration to practice in a related discipline in Michigan

In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health.

Strong organizational, planning, problem-solving, decision-making, and leadership skills required.

Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied backgrounds.

Ability to direct and manage a multi-disciplinary team of home health care clinicians, both professional and para-professional.

Computer skills and ability to use word processing, databases, spreadsheets, applications, etc. preferred.

Proof of current auto insurance coverage, Driver's License, and CPR certification.
